Commissioning large packages of care often with 24 hour provision or navigating a staff team for healthcare provision at home, can be a challenging prospect. The changes ahead will impact everyday life for both the service user and the people around them. Concerns about the capability and chemistry of the nurse or carer, or around securing funding can raise levels of anxiety.
So, finding just the right person, ensuring the right care package and then delivering them in a timely manner is crucial. We know what a huge difference that this can make. Bringing clarity and simplicity to what can be complex processes – and working holistically – considering practical and social needs alongside medical provision, help us to make that difference.
Where needed, we can help to create a seamless process from assisting with setting up Personal Healthcare Budgets (PHBs) to setting up new Commissioned packages and providing a link in communication between Healthcare and Social Care teams.
