The transition to new healthcare provision can appear daunting, with new providers, people, systems, and paperwork. Young people can often feel vulnerable or afraid – often having faced huge health challenges already – at the thought of moving on from trusted care providers or systems they have known over a long period of time.
Our knowledgeable team will support and walk you through this whole process, to ensure the right care and resourcing as you move from one set of services to another. With our staff by their side our service users are fully prepared for the changes ahead, as we start the process as early as needed to ensure this. We are experienced at working with commissioners and families alike to support from both sides with this transition.
We always include both the young person and their families in the process so that they’re empowered and informed at every stage. We want them to be confident in communicating about their condition and their healthcare needs. We guide them through any choices they may need to make and act as a valuable advocate with all the relevant health and social care teams.
